Evergreen’s commercial MVR costs $11.95 plus the license state’s fee. MVR + CDLIS costs $14.95 plus that fee. Both are one-off employer purchases for CDL drivers, not subscriptions.

What you pay at checkout
| Product | Report charge | Added at checkout |
|---|---|---|
| Commercial MVR, without CDLIS | $11.95 | Issuing state’s DMV fee |
| Commercial MVR + CDLIS | $14.95 | Issuing state’s DMV fee |
Use the state that issued the driver’s current license—not the employer’s location. State fees are passed through without markup. Online ordering is not available for every license state; use the checkout’s state selector to check availability.
Two example totals
These are Evergreen checkout quotes checked October 2, 2026, not prices for buying a personal record directly from a DMV.
| License state | State fee | MVR total | MVR + CDLIS initial total |
|---|---|---|---|
| California | $2.00 | $13.95 | $16.95 |
| Texas | $6.50 | $18.45 | $21.45 |
Fees can change. The current quote shown before payment controls; the examples are not a nationwide flat price.
Can CDLIS add costs later?
Yes. If the search finds additional licenses, Checkr automatically runs additional MVRs. The $14.95 report charge covers CDLIS and the initial MVR—not every additional record.
At checkout, you separately authorize saving your payment method and one later charge for this order’s verified additional Checkr and DMV costs, without markup. The amount depends on the extra records and fees. It may be charged after report delivery; you receive an itemized receipt. This is not authorization for other orders or a subscription.
MVR vs. CDLIS vs. PSP explains the scope difference before you choose.
Compare the total, not just the headline price
Check whether another quote includes the same report, state fees, additional-license searches, and any subscription or minimum purchase. A low advertised report price may not be the final total.
You do not need to sign in to buy. The driver still completes authorization, and you sign in to retrieve the report. See the ordering steps.
