Ohio CDL MVR for employers
Order a commercial MVR for a CDL driver licensed in Ohio. The total includes Ohio’s $5.00 fee at cost. The driver enters their details and authorizes the report privately, and you don’t need an account to buy.
What a Ohio MVR shows
An Ohio driving record lists moving-violation convictions, suspensions, and every crash the driver was involved in, regardless of fault. The BMV doesn’t decide who caused a crash; a conviction dated the day of the crash suggests fault. Source
Ohio keeps convictions and crashes on the standard record for 36 months. When an employer of a CDL holder requests the record for that purpose, the BMV provides at least 10 years of information, if available, plus medical card information. Source
The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
Ohio rules employers should know
- Ohio sends a warning letter at 6 points within 2 years. At 12 or more points within 2 years, the license is suspended for 6 months, and reinstatement requires a remedial driving course and retesting. Source
- Ohio Revised Code 4501.27 bars the BMV from releasing personal information except for listed uses, including an employer, or its agent or insurer, verifying a CDL holder’s information. Source
- Ohio Revised Code 4506.13 directs the BMV to provide CDLIS records to a motor carrier that is a current or prospective employer of the CDL holder. Source
- Ohio employers can request a one-time National Driver Register check on Form BMV 2151, signed by the driver and notarized. It shows only which states reported the driver in the past 3 years; you then order records from those states. Source
When you need a Ohio MVR
- Hiring: within 30 days of a driver’s start date, get an MVR covering the past 3 years from every state where the driver held a license or permit in that time. That includes Ohio even if your company is based somewhere else. 49 CFR 391.23
- Every 12 months: get an MVR covering at least the past 12 months from each state where the driver held a CDL or permit during that time, and review it. 49 CFR 391.25
- Not sure where else the driver has been licensed? MVR + CDLIS searches the national CDL database for licenses in other states.
Ordering directly from the Ohio Bureau of Motor Vehicles (BMV)
Employers can order certified records online or by mailing Form BMV 1173 with $5 per record, checking the purpose for employers of CDL holders and giving the company’s tax ID. One form can cover several employees with an attached list. Source
Drivers who want a copy of their own record should request it from the Ohio Bureau of Motor Vehicles (BMV). Evergreen sells reports to employers only. bmv.ohio.gov
Questions
- How much does a Ohio MVR cost?
- $14.25 for an MVR only, or $17.25 with a CDLIS search: Evergreen’s $9.25 or $12.25 report price plus Ohio’s $5.00 fee at cost.
- How far back does a Ohio MVR go?
- Ohio keeps convictions and crashes on the standard record for 36 months. When an employer of a CDL holder requests the record for that purpose, the BMV provides at least 10 years of information, if available, plus medical card information. The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
- Does the driver have to consent?
- Yes. After you pay, the driver gets a private link to enter their details and sign the authorization. The report runs only after the driver authorizes.
- Is a Ohio MVR enough for a driver qualification file?
- It covers the Ohio part of the record. A driver qualification file also needs records from any other state where the driver held a license in the past 3 years, plus the other documents listed in 49 CFR 391.51. An MVR is not a hiring decision.
