Florida CDL MVR for employers
Order a commercial MVR for a CDL driver licensed in Florida. The total includes Florida’s $10.00 fee at cost. The driver enters their details and authorizes the report privately, and you don’t need an account to buy.
What a Florida MVR shows
Florida’s 3-year and 7-year driver history records show license issuances, guilty traffic dispositions in that window, crashes where a citation was issued, and all open suspensions, revocations, cancellations and disqualifications. The complete record also includes dispositions where adjudication was withheld. Source
Florida sells 3-year, 7-year and complete records. On the complete record, alcohol-related entries stay for 75 years, and serious CDL violations can stay 10 or 55 years, depending on the offense. Source
The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
Florida rules employers should know
- Florida suspends a license for up to 30 days at 12 points within 12 months, up to 3 months at 18 points within 18 months, and up to 1 year at 24 points within 36 months. Source
- Florida CDL holders can’t take a driver improvement course to avoid points on a ticket, even one they got in a personal vehicle. Source
- Anyone requesting a record on Florida’s Form HSMV 90511 swears under penalty of perjury that they’re entitled to it, and may not redisclose it except as state law and the federal Driver’s Privacy Protection Act allow. Source
When you need a Florida MVR
- Hiring: within 30 days of a driver’s start date, get an MVR covering the past 3 years from every state where the driver held a license or permit in that time. That includes Florida even if your company is based somewhere else. 49 CFR 391.23
- Every 12 months: get an MVR covering at least the past 12 months from each state where the driver held a CDL or permit during that time, and review it. 49 CFR 391.25
- Not sure where else the driver has been licensed? MVR + CDLIS searches the national CDL database for licenses in other states.
Ordering directly from the Florida Department of Highway Safety and Motor Vehicles (FLHSMV)
Employers can mail Form HSMV 90511 to FLHSMV’s Bureau of Records, entering the exemption number for employers of CDL holders to receive the driver’s personal information, with a check or money order for the fee. Source
Drivers who want a copy of their own record should request it from the Florida Department of Highway Safety and Motor Vehicles (FLHSMV). Evergreen sells reports to employers only. flhsmv.gov
Questions
- How much does a Florida MVR cost?
- $19.25 for an MVR only, or $22.25 with a CDLIS search: Evergreen’s $9.25 or $12.25 report price plus Florida’s $10.00 fee at cost.
- How far back does a Florida MVR go?
- Florida sells 3-year, 7-year and complete records. On the complete record, alcohol-related entries stay for 75 years, and serious CDL violations can stay 10 or 55 years, depending on the offense. The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
- Does the driver have to consent?
- Yes. After you pay, the driver gets a private link to enter their details and sign the authorization. The report runs only after the driver authorizes.
- Is a Florida MVR enough for a driver qualification file?
- It covers the Florida part of the record. A driver qualification file also needs records from any other state where the driver held a license in the past 3 years, plus the other documents listed in 49 CFR 391.51. An MVR is not a hiring decision.
