Arizona CDL MVR for employers
Order a commercial MVR for a CDL driver licensed in Arizona. The total includes Arizona’s $6.00 fee at cost. The driver enters their details and authorizes the report privately, and you don’t need an account to buy.
What a Arizona MVR shows
Arizona’s MVD releases several driver record types, including an uncertified 39-month record, a certified 5-year record, a certified extended history and a commercial driver license record. Source
Arizona’s uncertified driver record covers 39 months and its certified record covers 5 years. A certified extended history record is also available. Source
The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
Arizona rules employers should know
- Arizona can require Traffic Survival School or suspend a license when a driver gets 8 or more points in 12 months. A DUI or reckless driving conviction carries 8 points; speeding carries 3. Source
- Arizona Revised Statutes § 28-455 limits the release of personal information from MVD records, with an exception for an employer, or its agent or insurer, verifying a CDL holder’s information. Source
- Employers can order a National Driver Register check from MVD on Form 96-0116 for $5. The driver signs in front of a notary or MVD agent, and the results go only to the employer. Source
- Arizona’s CDL manual tells drivers to notify their employer within 30 days of a traffic conviction other than parking, and within 2 business days of a suspension, revocation, cancellation or disqualification. Source
When you need a Arizona MVR
- Hiring: within 30 days of a driver’s start date, get an MVR covering the past 3 years from every state where the driver held a license or permit in that time. That includes Arizona even if your company is based somewhere else. 49 CFR 391.23
- Every 12 months: get an MVR covering at least the past 12 months from each state where the driver held a CDL or permit during that time, and review it. 49 CFR 391.25
- Not sure where else the driver has been licensed? MVR + CDLIS searches the national CDL database for licenses in other states.
Ordering directly from the Arizona Department of Transportation Motor Vehicle Division (ADOT MVD)
Employers can request records from MVD with Form 46-4416, by mail or in person. The requester’s signature must be notarized or witnessed by an MVD agent. MVD charges $3 for an uncertified record and $5 for a certified one. Source
Drivers who want a copy of their own record should request it from the Arizona Department of Transportation Motor Vehicle Division (ADOT MVD). Evergreen sells reports to employers only. azdot.gov
Questions
- How much does a Arizona MVR cost?
- $15.25 for an MVR only, or $18.25 with a CDLIS search: Evergreen’s $9.25 or $12.25 report price plus Arizona’s $6.00 fee at cost.
- How far back does a Arizona MVR go?
- Arizona’s uncertified driver record covers 39 months and its certified record covers 5 years. A certified extended history record is also available. The years of history in a report ordered through Evergreen depend on what the state releases for employment checks.
- Does the driver have to consent?
- Yes. After you pay, the driver gets a private link to enter their details and sign the authorization. The report runs only after the driver authorizes.
- Is a Arizona MVR enough for a driver qualification file?
- It covers the Arizona part of the record. A driver qualification file also needs records from any other state where the driver held a license in the past 3 years, plus the other documents listed in 49 CFR 391.51. An MVR is not a hiring decision.
