Annual MVR review form
A free, printable record of the yearly driving-record review that 49 CFR 391.25 requires for each driver. Fill it in on screen, or print it blank. Nothing you type leaves your browser.
What the annual review requires
- At least once every 12 months, get an MVR covering at least the past 12 months from each state where the driver held a commercial license or permit during that time. 49 CFR 391.25(a)
- At least once every 12 months, review the record to decide whether the driver meets minimum requirements for safe driving or is disqualified. Consider violations of the safety and hazmat rules, the accident record and traffic violations, giving great weight to speeding, reckless driving and driving under the influence. 49 CFR 391.25(b)
- Keep each MVR and a note with the reviewer’s name and the review date in the driver qualification file. Both may be removed 3 years after the review. 49 CFR 391.51
- The driver no longer has to sign a separate annual list of violations; that rule, 49 CFR 391.27, has been removed.
Getting the MVRs
Order a commercial MVR for each driver, or upload a CSV for 2 to 100 drivers. Totals for every license state are on MVR cost by state. For alerts between reviews, see MVR Monitoring; to hand off the whole driver file, see driver qualification management.
